I give permission for photographs of my child participating on the camps to be used in future camp marketing. I consent to any emergency medical treatment necessary during my child's time with the UKSA. I authorise the management to sign any written form of consents required by the hospital authorities if the delay in getting my signature is considered by the doctor to endanger my child's health and safety.

Undertaking: I understand that the UKSA / EGHA Hockey Academy LLP nor its agents or employees are under any liability whatsoever in respect of personal loss or damage or injury, howsoever rising, to the above-named person, whilst taking part in any of the academies activities

UKSA Refund and Cancellation Policy

When you have completed an online booking form, you have made a formal agreement to attend the camp(s) and we will allocate you a space on the camp accordingly.

If you need to cancel your place, our refund policy is as follows:

Up to 14 calendar days before the camp start date - 100% refund
Between 7 - 10 calendar days before the camp start date - 50% refund
5 calendar days or less before the camp start date - 0% refund
All refunds will be subject to a £5 administrative fee.

N.B Any cancellation or refund request must be submitted in writing by email or by telephone. Please note that we use the date that the email/call is received.

If a booking is cancelled by you or UKSA cancels a camp and payment has been made using Childcare Vouchers, a credit only will be offered for a subsequent camp.

If cancelling a camp due to illness or injury you will be entitled to a full refund (regardless of notice period) when providing a valid doctor's note, other matters can be discussed further by phoning 07908 189 115 or emailing wendy@uksportsacademy.com

If the UKSA cancels a camp, we will offer either a credit for a subsequent camp or a refund. In the event of adverse weather, the UKSA will endeavour to organise alternative indoor activities. Any credits issued will lapse 6 months from the date of the camp that it relates too.

All camps are subject to demand. Bookings must be made in advance. Payment must be made at the time of booking. The UKSA reserves the right to cancel a camp as necessary. I have read, understood and agree to abide by the above.
